2009-2010 Basketball Leagues (Parents)

Parents:


If your son/daughter is a member of a Greek Orthodox Church, ask his/her parish priest if the Church will have a team.

If your Church is NOT participating in the Leagues this year, contact other Greek Orthodox Churches in your area and see if they will have a team.


If your son/daughter is on a Church Team and he/she is UNDER 18 years of age, please follow these 4 easy steps:


Step 1) Download the Youth Registration Insurance Waiver Form (under 18) | Click here


Step 2) Fill out the Form


Step 3) Attach a copy of your Insurance Card


Step 4) Send in the Completed Form (with a copy of the Insurance Card) to the following address:

                 Brookline, MA 02445


If your son/daughter is on a Church Team and he/she is 18 years of age or older, please have him/her follow these 4 easy steps:
